About Our Service

Our social support individual services help you to remain social and take part in community life. These services are proudly offered by our In Home & Community Team. Keeping socially active is an important part of your daily routine and we proudly support your social connections as part of your general health and wellbeing.

Assistance to our clients is varied and comprehensive. We offer choice and control and our aim is to provide the necessary supports that empower our clients to live independently at home. Social support individual is an accompanied service supporting you to meet your recreational and social needs. Whether you prefer an established routine and need some help accessing community events or would like social visits at home from one of our friendly care staff, because everyone’s social needs are unique we tailor our services to suit you.

We provide safe and comfortable transportation and our personalised one-on-one service allows you to run your personal errands, enjoy a coffee and chat, explore the local community, stay active or feel supported when attending your doctors or medical appointments. If you’re not sure what you want to do, we can help you to locate new activities and research what’s available locally.

We offer our clients a small and consistent team of carers who take the time to get to know you, and we offer our services with a smile and caring heart. We are recognised for our personal touch and ability to create trusting and reliable relationships. Because everyone’s social needs are unique, we tailor our services just for you.

Our In home & Community team will support you to make informed choices about your care and the many services we offer, as advocates to support you to live the life you choose. We value your respect to privacy, and we have a culture of inclusion and we will treat you with dignity and respect. Our promise is to partner with you to help you feel comfortable, safe and build your independence.
